Kognitiv svikt och sömnbesvär – finns det ett samband och vad kan distriktssköterskans hälsofrämjande arbete bidra med?: En kvantitativ studie med material från Swedish National study on Aging and Care – Blekinge

Abstract [sv]

Bakgrund: Den äldre befolkningen i Sverige ökar konstant, och förväntas öka ännu mer framöver. Med detta ökar även andelen personer med kognitiv svikt samt andelen personer med sömnbesvär. I studier råder delade meningar kring om det finns ett samband mellan sömnbesvär och kognitiv svikt eller inte. Detta visar på att ytterligare forskning bör göras inom området. För att förebygga ohälsa är det även av stor vikt att belysa de riskfaktorer som finns kopplade till såväl sömnbesvär som kognitiv svikt, för att stärka det vetenskapliga underlaget till distriktssköterskans hälsofrämjande arbete för att kunna förebygga ohälsa ochfrämja hälsa hos den äldre befolkningen.

Syfte: Syftet med studien var att undersöka om det fanns ett samband mellan kognitiv svikt och sömnbesvär hos personer över 60 år samt vilka riskfaktorer som kan ha en påverkan på kognitiv svikt och sömnbesvär för att förbättra distriktssköterskans hälsofrämjande arbete.

Metod: Studiens design var en kvantitativ studie i samarbete med Swedish National study on Aging and Care - Blekinge (SNAC-B). Urvalet var personer över 60 år. Variabler som ansågs relevanta för studien valdes ut, såsom kön, levnadsvanor, sömnvanor och Mini-Mental State Examination (MMSE). Analys och bearbetning av data gjordes i SPSS. Analysen började med deskriptiv statistik över materialet och följdes av ett Mann-Whitney U Test för att se om ett eventuellt samband förelåg mellan sömnbesvär och kognitiv svikt. Slutligen gjordes Bivariat regressionsanalys och logistisk regressionsanalys för att kolla på vilka faktorer som hade påverkan på sömnbesvär respektive kognitiv svikt.

Resultat: Studien visade inget statistiskt samband mellan sömnbesvär och kognitiv svikt. Det gick att se att vissa livsstilsfaktorer har stor betydelse, främst för kognitiv svikt.

Slutsats: Inget samband påvisades mellan kognitiv svikt och sömnbesvär, däremot gick det att se att vissa levnadsvanor, såsom hög alkoholkonsumtion och låg fysisk aktivitet, har ett signifikant samband med kognitiv svikt. Genom ökade kunskaper inom ämnet kan distriktssköterskan bedriva ett preventivt och hälsofrämjande arbete

Abstract [en]

Background: The elderly population in Sweden is constantly increasing and is expected to increase even more in the future. This leads to an increase in people with cognitive impairment and people with sleep problems. In studies, there are some divided opinions about whether or not there is a connection between sleep problems and cognitive impairment. This shows that further research is needed in the area. In order to prevent ill health, it is of great importance to look at the risk factors that are linked to both sleep problems and cognitive impairment, in order to strengthen the scientific basis for the district nurse’s healthpromotion work in order to be able to prevent sleep problems and cognitive impairment andpromote healthy lifestyle in the elderly population.

Aim: The aim of the study was to investigate whether there was a relationship between cognitive impairment and sleep problems in people over 60 years of age, as well as if riskfactors may have an impact on cognitive impairment and sleep problems in order to improve the district nurse's health promotion work.

Method: The design was a quantitative study in collaboration with the Swedish Nationalstudy on Aging and Care - Blekinge (SNAC-B). The sample was people over the age of 60, and other variables relevant to the study were selected, such as gender, lifestyle related questions, sleeping habits, and Mini-Mental State Examination (MMSE) scores. Analysis and processing of data was done in SPSS. The analysis began with descriptive statistics of the data and was followed by a Mann-Whitney U Test to see if a possible connection existed between sleep problems and cognitive impairment. Finally, bivariate regression and logistic regression analysis were performed to check which factors had an impact on sleep problems and cognitive impairment.

Results: The study showed no statistical relationship between sleeping problems and cognitive impairment. It was seen that certain lifestyle factors are of great importance.

Conclusion: No connection was demonstrated between cognitive impairment and sleep problems, however, it was possible to see that certain lifestyle habits, such as high alcohol consumption and low physical activity, have a significant connection with cognitive impairment. Through increased knowledge in the subject, the district nurse can carry out preventive and health-promoting work.
